COVID-19 could directly affect the heart – Medical News Today

A recent stem cell study has shown that SARS-CoV-2, the novel coronavirus, can infect heart cells via the same receptor present in the lungs. This may be responsible for the cardiac complications associated with COVID-19.

Experts initially thought that COVID-19 was a respiratory disease, with symptoms including cough, shortness of breath, and pneumonia. However, more recent evidence into COVID-19 shows that the disease can also cause neurological and cardiac symptoms.

Physicians have reported changes to the circulatory system in people with COVID-19, sometimes leading to blood clots, as well as cardiac complications, such as changes to the heart rhythm, damage to heart tissue, and heart attacks.

Although there is widespread agreement that COVID-19 is a risk to the heart, whether these symptoms are due to the virus directly or a consequence of other disease processes, such as inflammation, has been unclear.

In a new study appearing in the journal Cell Reports Medicine, scientists have helped resolve this mystery by showing that SARS-CoV-2 can infect heart cells and change their function.

Their findings, from experiments in human stem cells, suggest that the cardiac symptoms of COVID-19 may be the direct result of the infection of heart tissue.

The scientists used a type of stem cell called induced pluripotent stem cells (iPSCs) to generate heart cells.

Scientists can create iPSCs from a persons skin cells and then reprogram them to become any cell type in the body. They provide a useful tool for research into human disease and a way to test new treatments.

In this study, the team programmed the iPSCs to become heart cells and later incubated them with SARS-CoV-2. Using microscopes and genetic sequencing techniques, the researchers found that SARS-CoV-2 could directly infect the heart cells.

They also showed that the virus can rapidly divide inside heart cells, which caused changes to the hearts ability to beat after a period of under 3 days.

We not only uncovered that these stem cell-derived heart cells are susceptible to infection by [the] novel coronavirus, but that the virus can also quickly divide within the heart muscle cells, explains first study author Dr. Arun Sharma, a research fellow at the Regenerative Medicine Institute of Cedars-Sinai Medical Center in Los Angeles, CA.

Additional experiments focused on the different genes expressed by heart cells before and after the virus infected them. These studies showed activation of the innate immune response and antiviral clearance pathways to help fight the virus.

However, how does the virus get into the heart in the first place? The researchers suggest that one way in which it gains access may be by using angiotensin-converting enzyme 2 (ACE2). This is the same receptor the virus uses to infect cells in the lungs.

Importantly, studies have shown that treatment with an ACE2 antibody can help stop SARS-CoV-2 from replicating and save cells in the heart.

By blocking the ACE2 protein with an antibody, the virus is not as easily able to bind to the ACE2 protein, and thus cannot easily enter the cell. This not only helps us understand the mechanisms of how this virus functions, but also suggests therapeutic approaches that could be used as a potential treatment for SARS-CoV-2 infection.

The researchers suggest that scientists could use stem cell-derived heart cells to screen new drugs and find compounds able to stop the infection of heart cells.

This key experimental system could be useful to understand the differences in disease processes of related coronaviral pathogens SARS and MERS, adds study author Dr. Vaithilingaraja Arumugaswami, an associate professor at the University of California, Los Angeles.

There are some limitations to this approach, however. These include the fact that stem cell-derived heart cells are not exactly the same as the real thing.

The researchers also studied the cells in a dish, an isolated system lacking the immune interactions that would occur in the human body.

Nevertheless, the experiments clearly showed that the cells became infected with SARS-CoV-2, which is in line with some clinical data showing the virus in the hearts of people who died from COVID-19.

Our Democrat-controlled Legislature and the powerful elite that run California hate it when we common, every-day voters use the power of the ballot box to turn away from their liberal agenda. In some cases, like the coming November 2020 election, we will all have the chance to reinforce our strongly held opinions that we do not like crime, higher taxes and government meddling in our employment, health care or living choices.

Will they listen this time? Doubtful.

California is one of the few states that have citizen-initiated ballot propositions. Unfortunately, the Legislature can also vote to put propositions before the voter. This year, we will have state propositions to consider in addition to the slew of local initiatives sponsored by our cities and the county. We have yet to see a complete list of the local issues you will be asked to consider, but you can be sure they will cost you money in the long run.

Proposition 14 is asking for your approval to issue a $5.7 billion bond to fund stem cell research dedicated to brain and central nervous system diseases, and conditions like Parkinsons, Alzheimers and strokes. A worthy endeavor to be sure. The kicker is this bond will end up costing California taxpayers $7.8 billion in principal and interest. Will this be another government-paid research program off which institutions profit? Why are you and I funding this research? Why not an investor group or big pharma?

Proposition 15 is asking us to substantially change the 40-year-old property tax protections we enjoy under Prop 13. One of the few breaks California property owners get is somewhat lower property taxes in a state where the cost-of-living is out of control. Proponents will give you the old, "This is for the schools" sales pitch. If this passes, and the projected billions are collected, the politicians will find a way to cheat it away from the schools in the next budget cycle like they always have. What Prop 15 will do is cause prices to rise and even more businesses to close and leave the state. A great post-COVID idea. Most city governments, realtors and, of course, unions have been bought off or threatened to garner their support for this job-killing proposal. You be the judge.

Proposition 16 will reverse our vote from 20 years ago to block affirmative action in California. It would legalize discrimination in university admissions, public employment or contracting. The message: it is OK to racially discriminate if you are the state.

Proposition 17 would give 40,000 formerly incarcerated individuals now on parole the right to vote. This will be framed as a "race issue" as two-thirds of our parolees are Latino or Black.

Proposition 18 will let 17-year-olds vote in primary elections if they will turn 18 before a general election. Gee, I wonder how teachers will ask their students to vote? I would prefer we raise the voting age to 30, at which point most of us have at least had a brush with real life.

Proposition 19 will make it easier for those over 55 to transfer their property tax base to a replacement residence. A rare gem of a good idea that helps people.

Proposition 20 is a return to the days of yesteryear when crime was punished. It makes theft crimes where the value is between $250 and $950 a felony. The relaxation of felony theft laws has led to skyrocketing numbers of property theft crimes in California because we stopped treating them as crimes. Yeah, for the good guys!

Proposition 21 is another try at convincing you that rent control is a good idea in a state with too little housing. It failed before. It will fail again.

Proposition 22 will reverse portions of Assembly Bill 5 which made independent contractors illegal as of January 1, 2020. Aimed at ride-share drivers and a host of other "gig economy" workers by the unions, AB5 caused an uproar when it passed. No matter your party, you do not want politicians defining how and when you can work for yourself.

Proposition 23 would require every dialysis clinic to have a licensed physician on-site. I am told this will mean the closure of clinics in areas like the Victor Valley, where physicians are in short supply and raise the cost for the other clinics who can comply.

Proposition 24 would toughen California consumer-privacy laws. Sound like a good idea? We are all tired of our information being sold and traded to internet companies like used baseball cards. The only reason I will vote no on this is that it creates a new state agency to monitor the regulations. Just what we need in California: Another state agency full of new, unionized state employees with fat pensions we must pay for. Good idea, poor execution.

Proposition 25 is another attempt to reign in the "catch-and-release" mentality running rampant in our criminal justice system. A yes vote on this measure keeps the current state law and cashless bail system in place. A no vote returns us to the previous system that requires assets or cash to secure bail set by the court.

The ballot initiative process in California is one of the few real opportunities left we ordinary voters can use to directly express our will. Do your own research and vote on these propositions that can change the future for California. Disregard the ads that are coming. Read the propositions.

HIV patient ‘first in remission’ without transplant – Japan Today

A HIV-positive man in remission may be the first patient effectively cured of the illness without needing a bone marrow transplant, researchers said Tuesday in a potential breakthrough.

HIV affects tens of millions of people globally and while the disease is no longer the automatic death sentence it once was, patients need to take medication for life.

In recent years two men -- known as the "Berlin" and "London" patients -- appear to have been cured of the disease after undergoing high-risk stem cell bone marrow transplants to treat cancer.

Now an international team of researchers believe they may have a third patient who no longer shows sign of infection after undergoing a different medicine regimen.

The patient, a 34-year-old Brazilian who has not been named, was diagnosed with HIV in 2012.

As part of the study, he was given several potent antiviral drugs, including maraviroc and dolutegravir, to see if they could help him rid the virus from his body.

He has now gone more than 57 weeks with no HIV treatment and he continues to test negative for HIV antibodies.

Ricardo Diaz, an infectious diseases expert at the University of Sao Paulo, said the patient could be considered to be free of the disease.

"The significance for me is that we had a patient that was on treatment and he is now controlling the virus without treatment," he told AFP.

"We're not able to detect the virus and he's losing the specific response to the virus -- if you don't have antibodies then you don't have antigens."

Diaz's findings were released as part of the first-ever all-virtual International AIDS Conference, held online this year due to the coronavirus pandemic.

The United Nations said Monday that 1.7 million people contracted HIV last year and there are now more than 40 million people living with it.

Diaz said his team's treatment method, which needs further research, was a more ethical avenue for gravely ill HIV sufferers than the bone-marrow transplant route.

"They come with a high mortality rate, there have been a series of patients who have either died from the procedures or it didn't work," he said.

Sharon Lewin, co-Chair of the International AIDS Society Initiative Towards an HIV Cure and director of the Doherty Institute for Infection and Immunity in Melbourne, said Diaz's findings were "very interesting".

She struck a note of caution however, due to the study's limitations.

She noted that the Brazil patients' antibody test had gotten weaker over time -- suggesting a diminishing immune response.

"This is very unusual to see in someone off antivirals," she said. "The Berlin and London Patients may be the only exceptions. This very provocative data needs more in-depth analysis."
