Private clinics’ peddling of unproven stem cell treatments is unsafe and unethical – Medical Xpress

Stem cell science is an area of medical research that continues to offer great promise. But as this week's paper in Science Translational Medicine highlights, a growing number of clinics around the globe, including in Australia, are exploiting regulatory gaps to sell so-called stem cell treatments without evidence that what they offer is effective or even safe.

Such unregulated direct-to-consumer advertising typically of cells obtained using liposuction-like methods not only places the health of individuals at risk, but could also undermine the legitimate development of stem cell-based therapies.

Many academic societies and professional medical organisations have raised concerns about these futile and often expensive cell therapies. Despite this, national regulators have typically been slow or ineffective in curtailing them.

As well as tighter regulations here, international regulators such as the World Health Organisation and the International Council on Harmonisation need to move on ensuring patients desperate for cures aren't sold treatments with limited efficacy and unknown safety.

So what's on offer?

Hundreds of stem cell clinics post online claims that they have been able to treat patients suffering from a wide range of conditions. These include osteoarthritis, pain, spinal cord injury, multiple sclerosis, diabetes and infertility. The websites are high on rhetoric of science often using various accreditation, awards and other tokens to imply legitimacy but low on proof that they work.

Rather than producing independently verified results, these clinics rely on patient testimonials or unsubstantiated claims of "improvement". In so doing these shonky clinics understate the risks to patient health associated with these unproven stem cell-based interventions.

Properly administered informed consent is often overlooked or ignored, so patients can be misled about the likelihood of success. In addition to heavy financial burdens imposed on patients and their families, there is often an "opportunity cost" because the time wasted in receiving futile stem cells diverts patients away from proven medicines.

The many recent reports of adverse outcomes demonstrate the risks of receiving unproven cell therapies are not trivial. In the USA three women were blinded following experimental "stem cell" treatment for macular degeneration (a degenerative eye disease that can cause blindness). One man was rendered a quadriplegic following a stem cell intervention for stroke. And a woman whose family sought treatment for her dementia died in Australia.

Other notorious cases involving the deaths of patients include the German government shutting down the X-Cell Centre and the Italian government closing the Stamina Foundation it had previously supported.

What's approved?

At present, the only recognised stem cell treatments are those utilising blood stem cells isolated from bone marrow, peripheral blood (the cellular components of blood such as red and white blood cells and platelets) or umbilical cord blood.

Hundreds of thousand of lives have been saved over the last half-century in patients with cancers such as leukaemia, lymphoma and multiple myeloma, as well as rare inherited immune and metabolic disorders.

A few types of cancer and autoimmune diseases may also benefit from blood stem cells in the context of chemotherapy. Different stem cells are also successfully used for corneal and skin grafting.

All other applications remain in the preclinical research phase or are just starting to be evaluated in clinical trials.

Often dismissed by for-profit clinics as "red tape" hampering progress, the rigour of clinical trials allows for the collection of impartial evidence. Such information is usually required before a new drug or medical device is released into the marketplace. Unfortunately, in the case of for-profit stem cell clinics, their marketing has gazumped the scientific evidence.

So what can be done?

Action is required on many fronts. Regulators at both an international and national level need to tackle regulatory loopholes and challenge unfounded marketing claims of businesses selling unproven stem cell interventions.

Researchers need to more clearly communicate their findings and the necessary next steps to responsibly take their science from the laboratory to the clinic. And they should acknowledge that this will take time.

Patients and their loved ones must be encouraged to seek advice from a trained reputable health care professional, someone who knows their medical history. They should think twice if someone is offering a treatment outside standards of practice.

The stakes are too high not to have these difficult conversations. If a stem cell treatment sounds too good to be true, it probably is.

Exciting progress is being made through innovative and carefully conducted research related to the potential of many types of stem cells for slowing MS disease activity and for repairing damage to the nervous system.

At present, there are no approved stem cell therapies for MS. There are different types of stem cells and the effects of receiving them depend on additional factors, including the specific procedures used to prepare and administer them, and biological conditions in the person to whom they are given. Stem cell therapy is still in the experimental stage, so its important for people to have the best available information to understand this exciting area of research and make decisions related to this complex issue.

In the U.S. and in other places around the world, for-profit stem cell clinics are appearing in increasing numbers. These clinics claim to have treated people with MS and people with many other disorders with stem cells. The scientific discoveries and innovations surrounding the potential of stem cell science have led to great enthusiasm about the benefits it could bring people.

The dark side of that enthusiasm is the hype, exaggerated publicity and inaccurate claims in the interest of financial gain.

People may develop unrealistic expectations of the benefits available from stem cell research and the speed with which they will be achieved.

Unfortunately, there will always be individuals and companies eager to take advantage of the necessary lag between research and clinical applications to offer the promise of so-called cures and therapies.

Many of these treatments are not based on sound scientific evidence, for example, this 2008 Stem Cell Network-funded study of the online marketing of 19 stem cell clinics found that the clinics claims of safe, effective and routine therapies were not substantiated by published evidence.

Hundreds of U.S. clinics are selling unapproved stem cell …

Hundreds of clinics across the United States are marketing unapproved stem cell treatments for conditions ranging from aging skin to spinal cord injuries, a new study finds.

In an online search, researchers found at least 570 clinics offering unapproved stem cell "therapies." They tend to be concentrated in a handful of states -- including Arizona, California, Colorado, Florida, New York and Texas -- but are scattered across many other states, too.

Most often, the clinics market stem cell procedures for orthopedic conditions, such as arthritis and injured ligaments and tendons. This does have science behind it, but is still experimental, medical experts said.

In other cases with little or no supporting evidence, clinics hawked stem cell "facelifts" and therapies for serious conditions such as chronic lung disease, Parkinson's disease and multiple sclerosis.

If these pricey stem cell treatments are unproven and unapproved by federal regulators, how can these clinics exist?

"I ask myself that question all the time," said Leigh Turner, a bioethicist who worked on the study.

Turner, an associate professor at the University of Minnesota's Center for Bioethics, said attention used to focus on "stem cell tourism" -- where people travel to countries such as China, India and Mexico to get unproven treatments.

"I think there's a misperception that everything here [in the U.S.] is regulated," Turner said. "But these clinics are operating here, and on a relatively large scale."

Stem cells are primitive cells with the potential to mature into various types of body tissue. Medical researchers have been studying the possibility of using stem cells to repair damaged tissue in a range of chronic ills -- with limited success so far.

But the general public has heard about the "promise" of stem cells for years, and it can be easy to be taken in by clinics' marketing tactics, Turner said.

Websites can, for instance, link to published medical studies that make their therapies seem legitimate, Turner said. "These businesses can be quite savvy," he said. "I think it's asking too much to just tell consumers to be wary. We need to be asking, why should these clinics be allowed to do this?"

Arthur Caplan, a bioethicist who was not involved in the study, cited some explanations for the growth of stem cell clinics.

The businesses are usually not engaging in interstate commerce, which helps them "fly under the radar," said Caplan, who directs the division of medical ethics at NYU Langone Medical Center in New York City.

Plus, he said, there's a regulatory gray area when it comes to so-called "autologous" stem cell therapy -- which refers to treatments that use a person's own stem cells.

"If you have cells from your own body reinjected, it isn't clear that you're getting a 'new biologic,' " Caplan explained.

Of the businesses Turner's team found, most marketed autologous therapies, usually using stem cells from people's body fat or bone marrow. But about one-fifth of the businesses claimed to use stem cells from umbilical cord blood or amniotic or placental tissue.

The issue goes beyond people wasting their money or having their "hopes dashed," Turner said. It's known some have been seriously harmed.

He mentioned two elderly patients in Florida who died following an unapproved stem cell procedure.

The U.S. Food and Drug Administration has taken steps against specific businesses. Last year, it sent a warning letter to a network of clinics that operate in California, Florida and New York. According to the FDA, the clinics illegally use stem cells from people's fat tissue to treat conditions such as Parkinson's, MS, amyotrophic lateral sclerosis (ALS) and autism.

"Many of these claims are outrageous," Caplan said. "These clinics are preying on vulnerable people."

His advice for consumers: "Be wary of any procedure that comes with celebrity endorsements or patient testimonials."

The FDA has issued draft guidelines on the use of stem cells. A public hearing is scheduled for later this year.

The new study findings appear in the June 30 issue of Cell Stem Cell.

First stem cell treatment for human administered in Atlanta …

Taking a landmark step, Atlanta doctors have injected millions of embryonic stem cells into a partially paralyzed patient, treating a human for the first time in the U.S. with the controversial research, officials said Monday.

The medical procedure took place Friday at an unknown local hospital and the person, who was not identified, later entered the Shepherd Center, which specializes in brain and spinal cord injuries, for rehabilitation.

While supporters hailed the treatment as a monumental medical advance, others derided it as a moral atrocity. There was some irony in Atlanta being selected as the first American site for embryonic stem cell treatment, considering there have been several legislative proposals previously calling for a research ban. Clearly there was a divide when the news was revealed.

This is a big deal for Atlanta, said Steve Stice, director of the University of Georgias Regenerative Bioscience Center. One of my sons roommates at UGA [who is paralyzed] could be helped by this eventually.

Atlanta was chosen for the initial clinical trial because of the citys reputation for cutting-edge research in spinal-cord injuries and the Shepherd Centers advanced rehabilitation center, said Anna Krassowska, spokeswoman for Geron Corp. of Menlo Park, Calif., which is sponsoring the research. The stem cell treatment could be offered to 10 patients at seven sites nationwide, with Atlanta receiving more patients, Krassowska said.

This clinical trial represents another step forward in Shepherd Centers involvement in an attempt to find a cure for paralysis in people with spinal cord injury, Dr. David Apple, the center leader for the stem cell procedure, said in a statement.

The Shepherd Center will play a large role in judging the success of the trial by closely monitoring whether the patient regains any sensation or movement.

If successful, the treatment could would mark a medical milestone and elevate Atlantas stature in the scientific community, providing a breakthrough in dealing with spinal cord injuries.

Stem cell research has drawn opposition because the process of harvesting the cells destroys human embryos, which some liken to an abortion. However, researchers see a medical need for the work, noting that embryonic stem cells can morph into any type of cell.

Georgians retain strong opinions on both sides of the issue, particularly in the political ranks.

Among the top candidates in the governors race, Democrat Roy Barnes supports stem cell research and Republican Nathan Deal opposes it.

I believe our state must embrace the necessity of stem cell and other types of bio-tech research as being essential to the development of a vibrant economy, Barnes said,

Deal, according to his spokesman Brian Robinson, supports research that does not include the creation of life for the purpose of destroying it.

State Representative James Mills, R-Gainesville, who sponsored a bill that allows parents to adopt human embryos, also spoke against the use of embryonic stem cells in research.

In a case where a childs life is taken in order for research to be done, I am opposed to that, he said.

The executive director of Georgia Right to Life, Nancy Stith, said she was shocked to learn the federal government had allowed the trial to proceed. We are very much heart-broken, disappointed and concerned that it is happening, she said.

Her organization supports stem cell technologies but is opposed to the research because it destroys human life.

When is it ethical that a human has to die to benefit someone else? Stith said. We believe never.

Stem cell research occurs at several Georgia institutions, including Georgia Tech, Emory University and UGA, and this particular procedure could bring more clinical trials to the area, UGAs Stice said.

Stice said embryonic stem cells have been tested in animals and he has heard of some clinics worldwide offering human therapies, but this clinical trial marks the first time the work has been approved by a government and carefully evaluated. The Food and Drug Administration gave its approval in July.

In this particular clinical trial, stem cells are converted into nerve cells that are injected into the damaged area of the spinal cord. It is hoped the stem cells will help repair nerve cells around the damaged area, potentially restoring movement.

The Geron Corp. said some paralyzed rats used in research regained the ability to walk, but otherwise was restrained in offering its expectations.

The Atlanta patient needed to be someone who was injured within the past 14 days and was between the ages of 18 to 65. The patient was injured in the middle section of the spine and was paralyzed from the waist down. No other information on the patient was released.

The surgery went well, said Krassowska.

The future of federal funding for embryonic stem cell research remains in question. A federal judge ruled in August that President Barack Obamas expansion of the research violated a federal law prohibiting taxpayer money be used for research that involves the destruction of human embryos. Geron did not receive federal funding for its work.

Company officials said they couldnt put a timetable on the process.

We just dont know, Krassowska said. This has never been done before in humans.

Larger, longer-term, controlled studies are needed to determine the safety and effectiveness of using stem cells to treat MS. When the results of these and subsequent clinical trials are available, it should be possible to determine what the optimal cells, delivery methods, safety and actual effectiveness of these current experimental therapies might be for different people with MS.

Outside of clinical trials, there are stem cell therapy clinics in the U.S. and around the world that claim to have treated people with MS and people with many other disorders with stem cells. However, none have provided medical evidence that their treatments work or are safe.

Many experts in the MS community have expressed concern that: In many countries these stem cell clinics are not held to strict sanitary guidelines and are allowed to operate without oversight over the safety of their procedures.

The sources of the stem cells they use are not always made clear, or the procedures they use to derive them or ensure they are free from infectious agents.

Safety of the procedure itself, and its long-term consequences, is a major issue. Areas of concern include the question of whether the cells could cause the severe immune attack known as graft-versus-host disease, or grow uncontrollably once inside the body and cause tumors or other serious problems. Another concern relates to whether follow-up care would be available if complications or other issues arise after a person gets home.

There is often no plan for how the safety, side effects and effectiveness of this experimental procedure will be measured and monitored over time.

Anyone who is considering stem cell therapy should evaluate carefully the potential adverse events that will be outlined in the consent form usually provided before medical procedures are performed.

How many clinics in Texas market stem cell procedures? What interventions do they promote? How many adult stem cell banks are located in Texas? How do they advertise their services?

There does not appear to be a comprehensive record of stem cell banks and clinics marketing stem cell procedures within the state of Texas. I am therefore trying to determine how many stem cell banks and clinics marketing stem cells in Texas can be found using an approach that a patient or customer might take when searching the Internet. In an effort to locate such businesses, I used Google search engine and entered such terms as stem cells Texas, stem cell clinics Texas, cosmetic surgery stem cell Texas, orthopedic surgery stem cell clinic Texas, and anti-aging stem cells Texas. In total, I found twenty businesses marketing what they describe as stem cell procedures as well as three stem cell banks. During my search I also noticed and recorded a spa marketing plant stem cells and a dentist who advertises dental stem cell storage. Most facilities market what they describe as adult stem cell procedures. However, I found one clinic advertising stem cells taken from amniotic fluid and another facility promoting bovine stem cells to prospective customers. While Im skeptical that Ive managed to find all such businesses, the list below documents at least some of the businesses currently banking adult stem cells or promoting stem cell procedures in Texas. If you are familiar with additional banks and clinics or identify any errors in my list please feel welcome to leave a comment and help me revise this list.
