BeyondSpring Announces Positive Topline Results from its PROTECTIVE-2 Phase 3 Registrational Trial of Plinabulin in Combination with Pegfilgrastim for…

NEW YORK, Nov. 16, 2020 (GLOBE NEWSWIRE) -- BeyondSpring(the Company or BeyondSpring) (NASDAQ: BYSI), a global biopharmaceutical company focused on the development of innovative cancer therapies, today announced positive topline data from its PROTECTIVE-2 Phase 3 registrational study showing that plinabulin in combination with pegfilgrastim met the primary endpoint with statistically significant improvement in the rate of prevention of Grade 4 neutropenia in Cycle 1 (31.5% vs 13.6%, p=0.0015), as well as achieving statistical significance in all key secondary endpoints, including duration of severe neutropenia (DSN) and absolute neutrophil count (ANC) nadir.

The PROTECTIVE-2 Phase 3 study is a double-blind, active-controlled, global study that enrolled a total of 221 patients. Patients in the trial were treated with docetaxel, doxorubicin and cyclophosphamide (TAC, Day 1 dose) in a 21-day cycle with plinabulin (40 mg, Day 1 dose) + pegfilgrastim (6 mg, Day 2 dose) versus a single dose of pegfilgrastim (6 mg, Day 2 dose). The primary efficacy endpoint was rate of prevention of Grade 4 neutropenia.

Plinabulin in combination with pegfilgrastim showed a statistically significant improvement compared to pegfilgrastim alone, with topline data summarized below. Data from all 221 patients were analyzed (combination arm n=111, pegfilgrastim arm n=110).

These data clearly demonstrate the potential for this combination to offer superior therapy compared to standard of care in the prevention of CIN, said Douglas W. Blayney, M.D., Professor of Medicine at the Stanford University School of Medicine and the global principal investigator for plinabulin's CIN studies. With current therapy, Grade 4 neutropenia still occurs in more than 80% of patients after chemotherapy, primarily in Week 1 after chemotherapy, which increases Emergency Room visits and hospitalizations due to infection and febrile neutropenia. Grade 4 neutropenia is also associated with increased mortality and reduced long-term survival due to reduction, delay, or interruption of chemotherapy. About Plinabulin in PROTECTIVE-2 (Study 106) CIN Study The Phase 3 portion of PROTECTIVE-2 is a double-blind and active controlled global study. It was designed to evaluate the safety and efficacy in breast cancer, treated with docetaxel, doxorubicin and cyclophosphamide (TAC, Day 1 dose) in a 21-day cycle with plinabulin (40 mg, Day 1 dose) + Pegfilgrastim (6 mg, Day 2 dose) versus a single dose of Pegfilgrastim (6 mg, Day 2 dose). TAC is an example of high febrile neutropenia risk chemotherapy; all G-CSF biosimilar studies use TAC in the pivotal studies.

Plinabulin and G-CSFs such as Pegfilgrastim are believed to have complementary mechanisms in preventing chemotherapy-induced neutropenia (CIN). This is a superiority study in CIN efficacy in the rate of prevention of Grade 4 neutropenia, comparing the combination head-to-head against Pegfilgrastim alone. Literature shows that the Grade 4 neutropenia rate for TAC and Pegfilgrastim at 6 mg is 83 to 93 percent, which presents severe unmet medical needs.

The absolute neutrophil count (ANC) data, which are used to calculate these endpoints, were obtained through central laboratory assessments by Covance Bioanalytical Methods using standardized and validated analytical tests. Covance was the clinical contract research organization (CRO) for patient recruitment and monitoring of global sites for this study.

About Chemotherapy Induced Neutropenia (CIN) Patients receiving chemotherapy typically develop CIN, a severe side effect that increases the risk of infection with fever (also called febrile neutropenia, or FN), which necessitates ER/hospital visits. The updated National Comprehensive Cancer Network (NCCN) guidelines expanded the use of prophylactic G-CSFs, such as Pegfilgrastim, from only high risk patients (chemo FN rate >20%) to intermediate risk patients (FN rate between 10-20%) to avoid hospital/ER visits during the COVID-19 pandemic. The revision of the NCCN guidelines effectively increases the addressable market of patients who may benefit from treatment with plinabulin, if approved, to approximately 440,000 cancer patients in the U.S. annually. Plinabulin is designed to provide protection against the occurrence of CIN and its clinical consequences in week 1, or early onset action after chemotherapy.

This year, Nov.17 has been designated World Cord Blood Day, an annual event to raise awareness for the life-saving benefits of cord blood transplants while educating parents, health professionals and the general public about the need to preserve these precious cells.

Cord blood transplants are being used to treat more than 80 different diseases and conditions including blood cancers like leukemia and lymphoma, neuroblastoma (the most common cancer in infants), bone-marrow failure disorders, inherited blood disorders and rare immune system disorders. They are also showing new promise for conditions that have never had treatment options, like autism and brain injury.

The first cord blood stem cell transplant, an international effort between physicians in the U.S. and Europe, was performed in France in 1988. Stem cells collected from a newborns umbilical cord blood were used to save the life of her brother, a 5-year-old with Fanconi Anemia. Since then, there have been more than 40,000 cord blood transplants performed worldwide.

Now standard of care for cancers of the blood and a host of other life-threatening diseases, blood forming stem cells for transplantation can be collected from bone marrow, circulating bloodor a newborn babys umbilical cord blood. Some experts believe cord blood contains nearly 10 times the number of stem cells found in bone marrow.

Because umbilical cord stem cells are less mature than adult bone marrow stem cells, they are also less likely to be rejected and can be used when there isnt a perfect match.

Between these threeoptions, the easiest collection by far is from umbilical cord and placental tissue after a baby is born and the umbilical cord has been cut, at no risk to mother or child, in a process that typically takes 5 to 10 minutes. To store your babys cord blood for use by your child and your family only, you make arrangements with a private cord blood bank ahead of delivery to collect and store the cells; the cost to you includes a collection fee of $1,500 to $2,000 and an annual storage fee of $100 to $125.

If you cant afford or dont wish to save your babys cord blood stem cells, you can donate them to a public cord blood bank at no cost to you or your family.

Its the equivalent of registering these potentially life-saving cells with the national bone marrow registry; they will be available to the families of other children who need to find a bone marrow match after a devastating diagnosis. Without information and education, however, 95% of all cord blood is discarded as medical waste.

Right now, there is no public cord banking option in Kentucky, although public cord blood banking is highly recommended by both the American Academy of Pediatrics (AAP) and the American Medical Association (AMA). There are fewer than 25 public or hybrid cord blood banks in the U.S., many limited to a specific geographic area. None of them include Kentucky.

The chances of finding a bone marrow match in your family are only about 25%, making the bone marrow and umbilical cord blood registries a lifeline in desperate situations. Odds are worse for African Americans and other ethnic minorities who are underrepresented on the registry and ethnicity matters in a bone marrow transplant.

Donating cord blood cells to a public bank adds to the library of cells that may save someones life and increases the chance of a match for all of us. Who benefits most? Children, patients with rare human leukocyte antigen (HLA) types and ethnic minorities.

We need to do two things: Make public cord blood banking an option in the commonwealth of Kentucky, and then encourage conversations between health care providers and expectant parents about preserving these life-saving cells.

There are 28 states with legislation that ask or mandates physicians to talk to expectant parents about cord blood banking. Kentucky is not one of them, but most of our surrounding states have such legislation in place.

Through a long-standing relationship between the National Stem Cell Foundationand world-renowned cord blood expert Dr. Joanne Kurtzberg, we have a path forward for training hospitals and collecting cells for storage at the Carolinas Cord Blood Bank (CCBB), one of the largest public cord blood banks in the world. Dr. Kurtzberg directs both the Pediatric Blood and Marrow Transplant (PBMT) program at Duke University and the CCBB.

She performed the worlds first unrelated cord blood transplant in 1993, paving the way for this now routine source of donor cells for children who need a bone marrow transplant and dont have a matched donor. She established the CCBB in 1998.

Doctors Consider Convalescent T Cell Therapy for COVID-19 – The Scientist

Physicians and researchers have had questionable success treating patients with severe COVID-19 with either antibody-based drugs or convalescent plasma. Neither of those strategies is a cell-based therapy, and, in a preprint posted October 26 on bioRxiv,researchers propose collecting SARS-CoV-2specific memory T cells from recovered individuals, banking the cells, and infusing them into patients as a treatment for infections.

There are data now that are coming out from the analysis of the immune response that are suggesting that the T cells are more important for protection than antibodies, says Antonio Bertoletti, who studies the development of immunological therapies at Duke-NUS Medical School in Singapore and did not participate in the work. There is therefore a rationale to use T cells to control the disease, he adds.

Previously, researchers had attempted memory T cell therapy to suppress cytomegalovirus and Epstein Barr virus. The recipients were leukemia patients who received a stem cell transplant to treat their cancer at the same time. It was this success, combined with the T cell findings from COVID-19 patients, that led Antonio Prez-Martnez, a pediatrician at Hospital Infantil Universitario La Paz in Madrid, and his colleagues to consider a cell-based treatment for the coronavirus.

The researchers isolated memory T cells, including some that released interferon gamma, a cytokine necessary for anti-viral immunity, upon exposure to SARS-CoV-2 antigens, from the blood of people who recovered from COVID-19. They showed that it was possible to freeze, store, and thaw these cells and propose that biobanks of memory T cells could be created from convalescent donors to be infused to treat disease. The practice of storing convalescent plasma, which includes antibodies but no immune cells, to treat COVID-19 patients is already in wide use, although with mixed data on its utility.

If you give plasma and antibodies, those antibodies could potentially fight COVID, but they are not produced in that person, so they are just have a normal half-life and they are cleared, says Rory de Vries, an immunologist at Erasmus University Medical Center in the Netherlands who did not participate in the study. But when T cells encounter a virus, they start expanding, and you might actually build your own new immunological memory while youre doing that, which will never happen with plasma.

Despite the theoretical potential, its not an easy approach, de Vries cautions. For one thing, the authors dont only store SARS-CoV-2 specific T cells, they store every memory cell, so that means that you also store your memory for measles or . . . previous vaccinations, he tells The Scientist. And if you infuse these T cells, I dont think we really know what might happen.

In a cell-based treatment, there are also concerns about immunological compatibility of the donor and recipient, just as there would be during an organ transplant, de Vries explains. The authors did characterize memory T cells based on human leukocyte antigens (HLA), molecules responsible for the immune systems ability to distinguish self and non-self. But de Vries cautions that HLA diversity is vast, potentially requiring more extensive characterization than the authors have done in the preprint to make the treatment safe.

According to Prez-Martnez, the HLA matching is sufficient because patients with severe COVID-19 have lymphopenia, a dearth of white blood cells called lymphocytes, a subtype of which are T cells. Lymphopenia is a biological marker of severe COVID-19, he says. The patients who are more lymphopenic have a greater chance of ending up in the ICU and eventually dying.

The lymphopenia triggered by the coronavirus is similar to the lymphopenia induced by chemotherapy drugs in patients before a stem cell transplant, so the transplant wont be rejected, he explains. This lymphopenic window during COVID-19 is the time at which it could be possible to replace a patients missing lymphocytes with SARS-CoV-2-responsive T cells from recovered donors. The idea is that lymphopenic patients dont have enough of their own T cells, so theyre not going to reject donor lymphocytes.

This strategy is complicated and needs some work, says Bertoletti. It could be that for a short time . . . when you are introducing these T cells in a patient, they will be basically able to suppress viral replication and kill the infected cells, he adds, but the unanswered questions of how important HLA matching is and what the appropriate timing for such a treatment complicates matters.

In order to test the safety of the treatment, Prez-Martnez and colleagues are beginning a Phase I clinical trial in patients hospitalized for COVID-19 with lymphopenia. In the transplant setting, doctors have not seen any side effects, he says, so I think that its worth it to try.
